As St. Elizabeth transitions to BMC Health System, please continue to access and request your medical records as you did before.
We are committed to keeping your Protected Health Information (PHI) private, and we are required by law to respect your confidentiality. If you would like a copy of your medical records, please click here to electronically request them or you can submit by fax. Our offices are opened from 8:00 a.m. to 5:00 p.m., Monday through Friday if you have any questions.

When you send a request to receive your medical records by fax, please be sure to provide a valid government-issued picture ID such as a driver’s license, identification card, passport, or comparison of signatures documented in the medical records.
If someone other than the patient is requesting to fax or mail the records, that person needs to provide a government issued picture ID and an original signed authorization letter from the patient. There may be a charge for creating a copy of your records.

On April 22, 2024, the Office of Civil Rights released a Final Rule which requires covered entities, like Boston Medical Center, Saint Elizabeth's Medical Center, and Good Samaritan Medical Center, and their business associates to obtain a signed attestation when they receive requests for PHI potentially related to reproductive health care. This attestation confirms the information will not be used or disclosed for prohibited purposes. It applies to requests involving health oversight activities, judicial and administrative proceedings, law enforcement, and disclosures to coroners or medical examiners.
